Cost is $2.50 per transcript. A signature is required. Follow the instructions below.

Visit the Office
Come into the BAS office and fill out a form. The fee is payable in person with cash, check, money order, or credit card information, and signature.

Fax Your Request
Send your request via Fax (510.644.6784) with the information listed below, plus your credit card number, expiration date, and signature.

Mail in Your Request
Mail in your request with the information below, plus check, money order, or credit card number and expiration date, and signature. No cash please! See address below.

Required Information
  • Date of request
  • Your last name, first name, birthdate, Social Security number
  • Name under which you registered for classes, if different from above
  • Your current telephone number, in case we have a question
  • Address where transcript is to be sent:
    xName of school, agency or company
    xAttn: (person's name)
    xStreet address
    xCity/State/Postal code/Country if not U.S.
  • Dates of attendance, from when to when
  • Program taken: GED, day classes, evening classes, medical, computers, etc.
  • Year graduated
Please note!
  • You must sign the request and pay $2.50 per transcript with cash (in person only), check, money order, or credit card.
  • If using a credit card, please provide the name on the credit card if different from the student's name, plus billing address, card number, expiration date, and signature.
  • Transcripts are official only if mailed to someone other than you. If mailed to you, the transcript will be unstamped and unofficial.
